Parents can go to Children\u2019s centres or a library for support, but all applications must be done on-line.<\/h5>\n<p>&nbsp;<\/p>\n<h5>All applications for travel support must include at least one piece of documentary evidence from the list below:<\/h5>\n<ul>\n<li>\n<h5>Statement of Special Educational Needs<\/h5>\n<\/li>\n<li>\n<h5>Education, Health and Care Plan<\/h5>\n<\/li>\n<li>\n<h5>S139 of Further Education students<\/h5>\n<\/li>\n<li>\n<h5>Proof of child\u2019s disability<\/h5>\n<\/li>\n<li>\n<h5>Medical evidence.<\/h5>\n<\/li>\n<\/ul>\n<h5><\/h5>\n<p>&nbsp;<\/p>\n<h5>These can be uploaded at the end of the application.
